My company, like many others, had an existing STOP THE JOB safety program that was designed to empower all employees to halt a job if they identified an unsafe condition or act. Cards were provided and filled out manually by the employee and turned in to their supervisor.  I saw an opportunity to automate this process and create a system to utilize the data from the cards to track and trend the results of our safety observations.  This would allow us to identify possible negative behavioral trends and address them before they had a negative impact on our employees, our equipment or our company.

Utilizing excel and VBA I was able to capture the inputs on pop up forms that would then transfer to a database as well as the safety card itself.  These cards could then be printed out and turned in as they had been previously.  Approximately 5 years later my company went to a similar electronic system company wide.  

These are the forms that the user could type there input into.  These cards could also be saved and the information carried over.
